RE: PRESCRIPTION BENEFITS/CARDS
THISMESSAGE WAS SENT FROM THE CITY OF NEWARK, DEPARTMENT OF PERSONNEL
Attention: City of Newark Employee Union Representatives
Re: Global Pharmaceutical Plan
Date: March 17, 2014
Please be advised that the City of Newark has made a business decision to terminate prescription services with Global Pharmaceutical Plan effective at 11:59 pm on March 31, 2014. All employees, retirees and family members will be affected by same.
Members will automatically be enrolled into Horizon Prime Therapeutics Prescription plan effective 4/1/2014. (This is the plan members enjoyed immediately prior to Global prescription plan.)
Toward a smooth transition, we ask that you assist us in relaying this change to your members. While there will be changes in GROUP numbers, the remaining information on the member’s old Horizon Prime Therapeutics card will be valid as of 4/1/2014; therefore those who are still in possession of their old cards can use them on that date.
Every attempt will be made to ensure that all subscribers receive their Horizon Prime Therapeutics prescription cards in a timely fashion. As of 4/1/2014, those who need their prescription information in order to fill immediate prescriptions can contact Horizon’s Pharmacy Member Services at 1.800.370.5088 or go online to www.horizonblue.com or call/visit the Employee Benefits Office for assistance as needed. In many cases, the member’s pharmacy will still have the member’s Horizon Prime information in their computer systems.
We appreciate your understanding and assistance as we make this transition/return to Horizon Prime Therapeutics Prescription Plan.
New Prescription Cards
Please be advised that all employees and eligible retirees will have a GLOBAL PHARMACEUTICALS prescription plan effective January 1, 2014. Cards and information for Global Pharmaceuticals are being mailed to your home address currently.
Effective 1/1/2014, please discard your previous Prescription cards (Horizon- Prime Therapeutics) and begin using the Global Pharmaceuticals card. When you visit your pharmacy on/after 1/1/2014, you will
need to take the new card to your pharmacy so that it can be added to your record in place of your old card.
Although your ID number has not changed, you cannot use your old prescription card after 1/1/2014. Your copayment amount will not change.
Information concerning your Global Pharmaceuticals plan is below and in the Open Enrollment Booklets for your information.
